中信递交签证材料行程单

中信银行代传递中有要求附上一份行程单，我自己写了一份，请大神指导一下这份行程单有没有大的缺陷:

　　Oct 5th Fisherman’s wharf, Palace of Fine Arts, back to Los Angeles
　　Oct 6th Beverly Center,Los Anegeles
　　Oct 7th take a flight from Los Angeles to Shanghai
