This document should contain parents signature and
should define the person in charge for the underage in Schengen area.

(Mandatory if the child is traveling alone or with only one of the parents.)请问出生证明公证可以是户口簿代替吗？另做了（未成年人出国父母同意书）包含了我们和孩子的关系以及父母同意说明，如果可以，户口簿就不用公证和认证了吧？
