后天德签，请教高人，这样的介绍信能过吗？

后天就德签了，请教坛子里的高手，这样的介绍信内容和格式能过吗？收入按年薪写会不会有问题（因其要求中写的是月薪）？后面领导签字的部分直接写成“DEPARTMENT MANAGER"可以吗？问题比较多，望指教，呵呵~~~~~
