请教如何填写预订拉古娜酒店的邮件！

我3月底去热浪岛想入住拉古娜酒店，由于英文不行，不知怎样填写预订酒店的邮件。请长老们帮帮忙。谢谢了！！！！！

只接受VISA / MASTERCARD 哦，只能帮你翻译了，具体是用中文填写还是英文填写，我觉得除了最后一栏签名栏需要中文填写外，其它的姓名最好用拼音，信用卡正面有你的姓名全拼。发卡银行的网站一般都有银行的英文名称。
有翻译不对的地方，还请高手们指正哦
我前几天刚定，但工行信用卡扣款老是不成功。。该开通的功能都开通了啊。。郁闷

这个Pages: 是填页数吗 还有 Ref: 是不是填写预定单号啊
Pax不是Fax哦
这里是指入住人数吧
